To receive payment from EarthScope, all vendors must complete the required vendor registration forms. These forms allow us to set you up in our financial system, ensure compliance with IRS requirements, and process payments accurately and on time.
This online form will satisfy the requirements for:
- Tax Information (W-9 or W-8BEN): Required for IRS reporting and to verify your taxpayer details
- Vendor Certification: Confirms that the information you provide is accurate and complete
- Contact Information: Ensures we can reach you with any questions or payment updates
- Payment Information: Allows us to issue payments correctly (e.g., ACH/direct deposit or check)
